Participant Applications

Dear Prospective High Hopes Participants, Parents, Caregivers and/or Teachers,

Thank you for your interest in High Hopes Therapeutic Riding, Inc.  Following are links to the various documents that High Hopes requires to be completed and returned to our office before you can be added to our waiting list.  When an opening becomes available, you will be contacted to schedule a pre-riding assessment.  A $50 assessment fee, payable in cash or check, is due at the time of assessment, prior to scheduled enrollment into the program.  Please note that we make every attempt to include new participants where possible.

In reviewing the following forms, you will find a Medical History form that will need to be completed, signed and dated by your physician. This form also contains a list of conditions that may suggest precautions or contraindications to therapeutic horseback riding.  Please review this list.  High Hopes requires that all participants be able to sit independently prior to participation.
